Tailor Made Support aims to meet your support needs, whatever they may be. We understand support means different things to different people, which is why we will work with you to design a personally tailored package to directly meet your requirements. We have a team of experienced, flexible and committed staff in various disciplines who are CRB checked. We also hold reviews to ensure you continue to receive the right support for you.

Outreach Service
Colebrook’s Outreach service provides a variety of support services for People aged 16+. We currently run 3 services within the Outreach Team, (listed below). The Outreach staff team have a wealth of experience in various fields and settings, are flexible and committed to their role. Alongside this all our staff are CRB checked.
1 – 'On Track' floating Support Service
The 'On Track' service is funded by Supporting People and as such follows the criteria set out by them. Therefore this Service is aimed at enabling vulnerable people over the age of 16 who require housing related support to access or maintain independent living, specifically those people who aren’t eligible for client specific services and are not linked to statutory services. We are unable to provide social support, personal care or counselling within this service, but will signpost for this if needed. support is provided by the ‘outreach team’ and is available 7 days a week between the hours of 8am-10pm

2 – New Learning Disability Framework
The Learning Disability Floating Support Service is aimed at enabling people over the age of 16 who require support to access or maintain independent living. The service can also offer a wide range of other support and can be based in the home or out and about in the community dependant or the individual’s needs or requirements. Support can be on-going and regular or a one off for specific reason such as a holiday
This service can also support people who want to maintain their current living situation or to develop and improve their ability to live independently.

3 – New Colebrook Domiciliary Care service
Finally, we have recently been registered by the Care Quality Commission to provide personal care. (CQC) We were assessed as having met the standard required and so now are able to provide a broader range of support.
- This can be solely personal care through our Domiciliary Care Service or as an add on to any of our other services – i.e. we can support people with personal care needs who may live in a supported housing project or receiving floating support from us.
